CASE FILE · ANONYMIZED · MIDWEST LTL
Six months from intervention threshold to audit-cleared.
A 120-unit Midwest LTL carrier reached us with the Unsafe Driving BASIC at 78 — well above the intervention threshold for fleets its size — and a focused HOS audit already on the docket. Three of their top shippers had quietly stopped tendering.
Two-track exposure: an active FMCSA investigation, and a commercial relationship with major shippers eroding in real time as those shippers pulled SMS Public reports during routine carrier qualification.
In the first 30 days we rebuilt 47 driver qualification files to current Part 391 standards, filed 9 DataQ challenges against inaccurate inspection records, and instituted a roadside-inspection coaching cadence keyed to the BASICs that mattered most.
Audit closed without a downgrade. Unsafe Driving BASIC fell from 78 to 41 across the next two SMS cycles. Two of three shippers returned to active tendering within the same quarter.
